1. Enrolment

I/We wish to enroll my/our child in Catholic Faith Formation classes with Lucan South Parish, which will be held from 4:45- 5:45pm every Saturday commencing from 13th September 2025 – June 2026.

These classes are open to children from Junior Infants through to Secondary 6th Year/Leaving Certificate.

Please note that our volunteers cannot administer any medication. Should your child require medication or intimate care, please discuss this with the volunteers who will work with you to establish how your child can be accommodated, according to relevant policies and procedures.

5. In cases of a medical emergency

In the event of illness or an accident, I/we give permission for medical treatment to be administered to my child, where considered necessary, by a suitably qualified medical practitioner and/or hospital. I/we understand that every effort will be made to contact me/us as soon as possible. In an emergency I/we can be contacted at the telephone numbers provided above.

7. Parental Consent / GDPR / Privacy

I/ We agree & consent to the following:

Allowing the above-named child/young person to attend the Catholic Faith Formation classes every Saturday (Sept 2025 – June 2026) from 4:45 to 5:45 pm in accordance with the permission granted by the child/young person.
Understand that there will be suitable supervision and an agreed code of behaviour while the child/young person is in the care of the volunteers,

In compliance with the guidelines for the Protection of Personal Data, in filling this form, I/ we agree and consent to the use of my personal data as well as my child’s personal data provided in the registration form specifically for the needs of the Catechists in the Catechetic Ministry. It may only be accessed by those with responsibility for managing records or group activities.

8. Code of Conduct

Respect for God and Sacred Teachings

Always show reverence for God, Jesus, and the Holy Spirit.
Treat the Bible, religious materials, and sacred objects with care and respect.
Participate in prayers and lessons with sincerity and attentiveness.

Respect for Teachers and Classmates

Be fair and honest in all our dealings with other children and leaders so as not to hurt any others by what we say or do
Listen attentively when the teacher or others are speaking.
Follow the instructions and directions of the activity leader.
Be supportive and encouraging to fellow students.

Responsibility and Participation

Arrive on time and depart at the appropriate time from the church activity
Actively engage in discussions, activities, and prayers.

Proper Behavior and Attitude

Maintain a positive and respectful attitude throughout the class.

Be kind to and treat each other with respect regardless of ability, ethnic origin, and cultural background.

Not engage in bullying of any kind and report any such behaviour if we become aware of it
Use appropriate language and gestures at all times.
Report any behaviour that makes us feel uncomfortable to the person in charge or someone designated for this purpose
Keep mobile phones on silent or switched off and left in a box designated for that purpose by the group leader

Respect for Church Property

Respect all church property and the property of others as well as taking care of our own.

Keep the classroom and church environment clean and orderly.
Handle church property, books, and supplies with care.

Safety and Courtesy

Follow safety guidelines within the church and classroom setting.
Be mindful of personal space and boundaries.
Remain in a supervised area at all times during the church activity
Not take audio or visual recording of anyone present at the church activity without the approval and monitoring of group leaders

Commitment to Faith and Learning

Show a willingness to learn and grow in faith.
Practice the values of love, kindness, and forgiveness as taught by Jesus.
Be an example of Christian behavior both in and outside of class.
